- If you have just completed making an online payment in support of the 2024 Coalition Conference, thank you! Please fill out the form below so we can make sure your support opportunity is accurately represented at the Conference.
- If an Invoice is needed: If your organization requires an invoice, please fill out the form as directed. After, be on the look out for an invoice sent to the email provided on the form.
- To Pay by Check, Venmo or Zelle: Fill out the form below and send in payment. Make checks out to AFFCNY and mail to the Coalition at 108 Main Street, Suite 5, New Paltz, NY 12561. You will receive an email from the Coalition with a receipt confirming your sponsorship and payment after it is received.
